Transcripts

In order to release your transcript, we need to have written authorization from you.

You may use our Transcript Request Form OR send a letter to:

100 Reservoir Road
Herkimer, NY 13350

In the letter specify whether you want an unofficial or official transcript, the complete address where the transcript is to be mailed, your social security number, your full name and current address, the name which your records are under, if different, and then sign it.

Enclose the appropriate fee, if needed. There is no charge for an unofficial transcript. Official transcripts are $5.00 each.

You may also compose your request and FAX it to the Registrar's Office at
315-866-1657. If you want an official transcript, you need to also include a Mastercard, Discover or Visa card number, the expiration date, and under whose name the card is registered, so that the fee can be applied.
